Complete these forms to comply with Wisconsin Department of Children and Families requirements.

Health History and Emergency Care Plan

Include important medical information and an emergency plan for your child’s safety.

Child Health Report 
Give details of your child’s health information. This will help us to ensure that he or she is properly cared for and safe at the daycare center.
